Test Application Requirements

Mid Ohio Chapter tests are filled on a first come, first serve basis.

A complete and accurate Test entry is REQUIRED to admit a dog into a test. A complete entry:

  • Must be submitted by the dog's owner

  • Must include the dog's NAVHDA Registration Number

  • Must include the owner's NAVHDA Member Number (located on the first line of your VHD magazine label)

  • The Handler's NAVHDA Number

  • A completed official Test Entry Form

  • Test Entry Fee payment

PLEASE NOTE: If your entry is incomplete, your dog will not be entered in the test until all required information is completed.

Unfortunately we can not reserve a spot for anyone without both an accurately completed test entry form and payment.

After Downloading, fill out the form and then click “SAVE AS” in the upper left corner or “PRINT” in the upper right corner of the test form. Please either email or mail your completed Test Entry to the Test Secretary:

Please note, no test entry is complete until a fully completed entry form and the appropriate test fee are received.

*** NAVHDA Dog Registration Number: All dogs entered in NAVHDA tests must be NAVHDA registered and have a NAVHDA registration number. This number begins with two letters identifying the breed and is NOT a person’s NAVHDA membership number. Neither is it a number issued by any other registry.

Effective July 1, 2018, it is a requirement that Owners (at least one owner of co-owned dogs) and Handlers of dogs must be NAVHDA Members in order to enter a NAVHDA test. The statement "...must be NAVHDA Members..." refers to the NAVHDA parent organization commonly known as NAVHDA International.

Important! Dogs are eligible for a Natural Ability Test until and including the day they reach 16 months of age. Dogs over 16 months may be tested for evaluation. No prize classifications are awarded dogs tested for evaluation. Dogs over 16 months of age may be tested for evaluation if space is available.

Test Entry Policies

Wait list:

If our test is full upon receiving your test entry and payment, you will be contacted immediately and your name will be placed on the wait list in the order that it was received. Those on the wait list who withdraw while still on the wait list or who are not entered into our test will receive a refund of testing fees they paid after the test dates. If you wish to be removed from the wait list, you must notify the Test Secretary in writing (email is acceptable) as soon as possible. Once a dog from the wait list is entered into the test, test withdraw policy will apply.

Test Transfer policy:

If you are on the wait list you may request to moved to the next Mid Ohio Chapter test if you are unsuccessful in gaining an entry into the original test. To transfer to the next Mid Ohio test you must submit in writing (email is acceptable) that you wish to transfer and submit an updated test entry form to the Mid Ohio Test Secretary. If you elect to transfer to the next Mid Ohio test, any test entry fees will be held as payment for the next test. If there is an available opening in next test you will automatically be entered in that test and the chapter test withdraw policy will apply.

Test Substitution policy:

You may request in writing (email is acceptable) to substitute either a dog or handler, but not both, more than 30 days before the test by submitting in writing the reason for the substitution and an updated test entry form to the Mid Ohio Test Secretary. You are permitted one substitution request per test entry of the same or lower level test. (Substitution of a NA entry for a UPT/UT is acceptable) Request’s to substitute a dog will be reviewed and approved by one or more of the Board of Directors and the Board of Directors decision will be final. If your request is denied, the chapter test withdraw policy will apply if you choose to withdraw from the test.

A handler substitution may be made less than 30 days before the test in cases of hardship. If time permits, a request should be made in writing and an updated test entry form submitted to the Mid Ohio Test Secretary. Request to substitute a handler will be reviewed and approved by one or more of the Board of Directors and the Board of Directors decision will be final. If your request is denied, the chapter test withdraw policy will apply if you choose to withdraw from the test.

Test Withdraw / Refund policy:

To withdraw a dog from the Mid Ohio test you must notify the Test Secretary in writing (email is acceptable) as soon as possible.

If a handler withdraws a dog from the test more than 30 days before the test, a 50% refund of paid test entry fees will be given if the test spot is filled by another entrant. Refunds will be issued after the test dates. If a handler withdraws a dog 30 days or less from the test date, NO REFUNDS WILL BE GIVEN except for a medical/health issue with the dog or handler. A handler may request a 50% refund for medical/health reasons in writing (email is acceptable) to the Mid Ohio Test Secretary, along with the reason they are unable to participate in the test. All request for refunds will be reviewed by one or more of the Board of Directors and their refund decision will be final.

Registration Certificate

NAVHDA provides a single document that serves as both the registration certificate and the dog’s pedigree. It is light brown in color and folds into fourths.

The certificate lists the name of the owner(s) as recorded by NAVHDA. Only the owner as listed in NAVHDA records will receive certificates, etc.

If you do not have the light brown certificate or the name on the certificate is not yours, you may need to register you dog or complete a "transfer of ownership."

  • The office is generally staffed during business hours Monday through Friday.

Note that with proper documentation the Central Office can issue a number under urgent conditions, e.g. in order to enter a test at the last minute.
